The
Service Coordinator assists Cambrian Center Residents to identify and
access other services that they want or need to maintain their
independence, live in their apartments, and improve their quality of
life. Essentially the Service Coordinator may act as an ombudsman
for a resident in discovering and utilizing health, education, cultural
or other activities available in Cambrian Center or the surrounding
community.
Typical examples
of the assistance might include helping to explain and
complete health insurance forms, assisting with obtaining or updating
Social Secutiry forms, helping those for whom English is a second
language to interface with the American Health Care system or
government agencies. And sometimes simple
legal forms can
appear
very complex. The Service Coordinator can help obtaining medical
equipment such as a walker, acquiring a telephone that addresses a
vision, hearing, or mobility impairment or reviewing documents such as
an Advanced Health Care Directive as well as making referrals to get
assistance at home. The Service Coordinator will "problem-solve"
with residents about things that are affecting the resident's
life and obtain the information necessary for them to make better
decisions about choices that may be ahead.
